** All required items will be marked with a (**) No request will be fulfilled if any of the required information is left out!
** copy & paste the form with your information, you may use the comment section or private messaging to do so, which ever you prefer, but I require the form!! ** you CAN remove the info in the parentheses when you transfer your info onto the form!
** No matter what font style you chose, I will provide you with 2 banner/options - if you do not like either font I used I will be glad to change it (as well as anything else) to make your vision come true!!
REQUEST FORM
(Basic Information)
1. Title **
- Subtitle (if you desire / also include if you want this the same font style as title)
2. Author/s Name/s **
3. Website **
4. Theme/Genre **
5. Main Character/s (only if you want them present on the banner/cover)
6. Reference Images - links only - (also mention any that you want to be on there, if not I will chose from your linked images or pick one myself!)
7. Black & White or Color **
8. Font ** (chose from any of these styles cursive/script, serif, sans serif, etc.) * if you're title is in all caps or all lowercase please mention that!
9. Additional Information **
(list any addition information, remember - the more I know about what you want your banner/cover to look like, the more accurate the end result will be to what you have in mind! Please mention specifics, including characters and additional aspects such as flowers, etc.. )
**** Creating your banner or cover may take at least 1 to 3 days, please remain patient! ****
-- I do not include my name anywhere on your banner/cover, I believe in credit but I do not want to put any distracting things on your stuff! I would appreciate it if you include credit to me somewhere in your story/author's notes/etc :) :)
